This Notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. We are required by law to maintain the privacy of your protected health information and to provide you with this Notice of our legal duties and privacy practices.

Your health information

Seth Premier Medical maintains records of the care we provide, including your name and contact information, your medical and health information, and billing details. This Notice applies to all records of your care generated by the practice.

How we may use and disclose your health information

We may use and disclose your health information, without your separate authorization, for the following purposes:

  • Treatment: to provide, coordinate, and manage your care among physicians, nurses, laboratories, pharmacies, specialists, and others involved in your treatment
  • Payment: to bill and collect for the care and services you receive, including membership fees and insurance billing
  • Health care operations: to support the quality, training, administration, and business operations of the practice
  • As required by law, and for public health and safety, health oversight, judicial and law enforcement requests, workers' compensation, research permitted by law, and to relatives or others involved in your care

Uses and disclosures that require your authorization

Most uses and disclosures of psychotherapy notes (if any), uses and disclosures for marketing, and any sale of your health information require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

Your rights regarding your health information

  • Request restrictions on how we use or disclose your information
  • Request that we communicate with you confidentially, by a specific means or at a specific location
  • Inspect and obtain a copy of your records
  • Request that we amend your information
  • Receive an accounting of certain disclosures
  • Receive a paper copy of this Notice on request
  • Be notified in the event of a breach of your unsecured health information

Our responsibilities

We are required by law to protect the privacy of your health information, to provide this Notice of our duties and privacy practices, and to follow the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for information we already have as well as information we receive in the future.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
